There needs to be some sort of symptom tracker for every patient. So, patient charts will be posted here and kept updated by the current Attending Physician.
REQUIRED INFORMATION FOR THE CHART
Location of incident:
Name of Attending Physician:
Name / occupation / age / sex of Patient:

Mis-diagnoses:
Rule violation(s) / ethical breaches (if applicable):
Attending's "Aha!" moment:
Patient's final diagnosis:
This is so that we can all keep track of the patient's status. If, for some reason, the case gets passed off to another Attending Physician, the change MUST be noted.
